## Service: Healthcare in Spain (public system, insurance, Barcelona hospitals)

The medical question on a move is settled by choosing a system, not a doctor.

**Two systems.** Spain runs a public health service and a private sector alongside each other. The public system is funded through contributions; access follows from your position in the Spanish social security system rather than from payment at the point of care, and it is strongest in complex and serious cases, oncology, transplant and emergency medicine. Private medicine is paid by policy or directly, buys short waits and a named doctor, and commonly offers consultations in English. Most relocating families end up using both.

**How public access actually arises.** Not bought on arrival, and not automatic from living in Spain. The order is: position in the social security system, padrón registration at the address, the targeta sanitària individual (TSI), then registration with the CAP, the primary care centre for that address. In Catalonia the procedure is set by CatSalut. A newborn has a separate TSI procedure with its own published guide.

**The Barcelona centres arriving families ask about**, with what each is known for rather than as a ranking:
- Hospital Clínic Barcelona — public university hospital with its own research institute and an international patient and traveller service.

**Where policies fail.** On a pregnancy that already existed when the contract was signed, and on a serious case inside an exclusion. Several months' waiting period on a birth or caesarean is common, and waiving other waiting periods on switching insurer does not necessarily extend to the birth. ## Service: Baby in Spain (birth, residence and citizenship for a child)

Moving to Spain on a Digital Nomad Residence often coincides with a child arriving. This programme holds one file from pregnancy planning to the child's Spanish citizenship application, instead of leaving the family to coordinate a clinic, an insurer, the Registro Civil, a consulate and the immigration authorities separately.

**The route.** Digital Nomad Residence, insurance and healthcare planning, pregnancy support, birth in Spain, birth registration, passport and consular documentation, health insurance for the newborn, Spanish residence for the child, the TIE residence card, residence period monitoring, and the citizenship application. Each step depends on the previous one, so documents produced out of sequence have to be produced again.

**Residence for the child.** Where one parent holds a Spanish Digital Nomad Residence, the child may be granted the corresponding residencia as a family member, subject to the applicable requirements. The UGE handles familiares de teletrabajadores de carácter internacional under Ley 14/2013; applications are filed electronically through the Ministry, and may be filed through a representative where the appropriate authority has been granted.

**The one-year rule, stated precisely.** Birth in Spain does NOT grant Spanish citizenship automatically in most standard situations with foreign parents. What it changes is the waiting period: under article 22 of the Código Civil, a person born in Spanish territory may apply for citizenship by residencia after one year of residence rather than ten. That residence must be legal, continuous and immediately preceding the application, so the date is calculated case by case against the child's own documents. No honest firm promises a Spanish passport a year after the birth; what can be held is the legal residence timeline and the point at which the entitlement to apply arises.

**Insurance, stated precisely.** Whether cover exists is decided by the insurer under the particular contract, never by an adviser. Where a pregnancy already existed when a policy was taken out, a number of medical policies apply a waiting period of several months to a birth or a caesarean, and a waiver of other waiting periods on switching insurer does not necessarily extend to the birth. Obtain the insurer's written position before making financial decisions on the strength of a policy.

**Fees.** Consultation EUR 150 with a personal roadmap. Pregnancy and birth concierge from EUR 850. Newborn documents and residence EUR 950. Spanish citizenship path EUR 1,350. An interactive checklist that answers the question "which documents do I need for the Spanish digital nomad residence permit" for one specific situation rather than in general.

There is no single universal document list. The required set depends on four things, which the tool asks in order:

1. Where the application is filed — from inside Spain through the UGE, or from abroad through a Spanish consulate.
2. The country where the employer is based or the professional activity is registered. Russia and Belarus are covered in full; other countries route to a manual review.
3. Employment type — employee of a company, freelancer or sole trader, or company owner.
4. Who applies alongside the main applicant — spouse, unmarried partner, minor children, adult dependent children, other dependents.

**Documents from the country of origin.** Employees: employment contract, employer letter, written permission to work remotely from Spain, proof of the employer's registration and trading activity, recent payslips, and bank evidence of salary arriving. Freelancers: proof of registered activity, contracts with foreign clients, evidence of how long the professional relationship has run, a client letter confirming remote work from Spain, the client company's registration documents, invoices and bank receipts. Company owners: incorporation documents, proof of the applicant's shareholding, documents on role and authority, company financials and proof of the applicant's own income. All applicants additionally need a criminal record certificate, a CV, and proof of either qualifying education or the required period of professional experience.

**Documents filed in Spain.** Full copy of the passport, the application form, the government fee, proof of lawful stay in Spain when filing from Spanish territory, the declaración responsable on criminal records, medical insurance where the chosen social security route requires it, and proof of sufficient funds. A rental contract and padrón registration are NOT automatically part of the initial UGE filing set — a common and expensive misunderstanding.

**Translation and legalisation.** The rule is set by the country that issued the document and the agreement in force between that country and Spain, not by the applicant's nationality. The same type of document issued in two different countries is legalised differently. Statements of the form "a birth certificate always needs an apostille" are wrong. Where the basis is not confirmed for a given situation, the checklist says so explicitly instead of applying another country's rule.

**Social security.** A social security agreement is in force between Russia and Spain, and in a suitable situation the applicant can evidence continued application of Russian social security legislation during temporary work from Spain with the corresponding official document. For Belarus the position is determined separately and depends on the international arrangements in force; the Russian procedure is not carried across to Belarusian applicants.

**Family documents.** Passports, marriage certificate or proof of registered partnership, birth certificates, proof of dependency for adult children and other dependents, and additional proof of funds whose amount scales with the number of people applying together.

**Progress tracking.** Each document can be marked as held, still to obtain, or needing correction, and the state is kept so the applicant can return to it over the weeks it takes to gather a file. The completion percentage is an indicator of how full the checklist is, not a legal assessment of the chances of approval.
