Two systems, and the choice between them
Spain runs a public health service and a private sector alongside each other. The public system is funded through contributions and access follows from your position in the social security system rather than from payment at the point of care; it is strongest in complex and serious cases, oncology, transplant and emergency medicine, and its university hospitals run their own research. Private medicine is paid through a policy or directly, buys short waits and a named doctor, and commonly offers consultations in English. For most families the practical answer is both.
How access to the public system arises
Access is not bought on arrival and does not follow automatically from living in Spain. It follows from your position in the Spanish social security system, and in Catalonia the procedure is set by CatSalut. The order is status, then padrón registration at your address, then the targeta sanitària individual (TSI), then registration with the CAP, the primary care centre for your address. A newborn has a separate procedure for the TSI, for which CatSalut publishes its own guide.

Hospital Clínic Barcelona is a public university hospital with its own research institute and a care service for international patients. Centro Médico Teknon is a private centre in the Quirónsalud group, organised into institutes for cardiology, oncology, neurosciences and pulmonology alongside gynaecology and obstetrics. SJD Barcelona Children's Hospital is one of Europe's principal children's hospitals, with maternal-fetal medicine, paediatric oncology and rare diseases. Dexeus Mujer specialises in gynaecology, pregnancy care and assisted reproduction.

Where a policy usually fails
A policy rarely fails on small things. It fails on a pregnancy that already existed when the contract was signed, and on a serious case inside an exclusion. A number of policies apply a waiting period of several months to a birth or a caesarean, and a waiver of other waiting periods on switching insurer does not necessarily extend to the birth. Whether cover exists is decided by the insurer under the particular contract, never by an adviser.
